Required Forms for Admissions

The following forms are required by "ShinyKidz", which must be completed by a parent or legal guardian and be returned by the requisite dates. Students will not be admitted unless all required forms are on file in the school office. Minimum age of the child should be 1 year 6 Months to 5years at the time of admission. Application form can be obtained from the Reception counter on any working day between 9:30am to 3:30pm.

 

Application Form

This form is completed as the initial request for enrollment. Parents need to keep the information updated by notifying the office of new information.

 

Health Record

 

Students enrolling in "ShinyKidz" must have a current immunization record on file in the office. New students must have a physical medical report signed by a physician before the admittance to the school.

 

Parents’ Details Form - This form provides the following: Emergency contact information (should be kept current) - a new Authorization Form must be completed each year.

 

Carpool/Pickup Authorization - This lists those individuals who are authorized to transport the child to and from the school. You may make changes to this form by coming into the office. A new Carpool/Pickup Authorization form must be completed each year. No child will be released to an unauthorized person.

Documents to be submitted at the time of admission:
Photostat copy of Date of Birth of the child & Photostat copy of Residence Proof.
4 passport sized photograph of the Child & 1 passport sized photograph of each Parent.

School Uniform:
During the time of admission, you will be required to buy a kit for your child which contains a school bag, water bottle, diary, a pair of uniform and stationery.
